FAA Member Information Form

Prior to the October 26th Information Meeting, we need to gather vital information in order to help each member accurately assess how the conversion will benefit them. Therefore, we ask that you complete the Member Information Form below. This information will remain COMPLETELY CONFIDENTIAL and will not be shared with anyone outside of the HUTTON staff. If we receive your completed survey prior to our initial Conversion Information Meeting, we will be able to generate an Individual Unit Analysis (IUA) which we will distribute to you at the meeting.
